How to schedule an appointment
What to bring to your appointment
New patients, please bring the following to your appointment:
- All medications that you are currently taking
- The name, phone number and address of your preferred pharmacy
- The name and phone number of your previous physician
- Medical records from your previous physician — please have these faxed, emailed or mailed prior to your visit
- Medical records from any emergency room visits within the last 12 months
- A form of identification with your picture (driver’s license, passport, matriculate, student ID)
- Proof of income
- Completed new patient forms (click here to download)
Returning patients, please bring the following to your appointment:
- All medications that you are currently taking
- A form of identification with your picture (driver’s license, passport, matriculate, student ID)
- Medical records from any emergency room visits since your last appointment
- Completed returning patient forms (click here to download)
Cancellation and reschedule policy
- We strive to be flexible and accessible for our patients, and recognize that sometimes patients will need to reschedule or cancel appointments.
- In order to best serve all our patients, scheduled appointments must be cancelled or rescheduled no less than 48 hours prior to the appointment during business hours.
- New patients who do not cancel or reschedule 48 hours prior to their appointment during business hours will be charged a $25.00 no-show fee.
- Returning patients who do not cancel or reschedule 48 hours during business hours prior to their appointment will be charged a $20.00 no-show fee at their next appointment.
Appointment costs and cancellation fees
First-time visit: $30.00
Regular appointment: $25.00
Well-women visit: $50.00
Appointment no-show or cancelation: $25.00
Cancelling an appointment
Call the Clinic during business hours, Monday to Thursday, 8:30 am – 12:30 pm; Friday, 8:30 am to 1:00 pm, 972-484-8444.
Or you may notify the Clinic via email using the form below. If you cancel via email, please call to reschedule as soon as possible.
All appointments must be canceled within 48 business hours prior to the date of the appointment. If the appointment is canceled with less than 48 business hours then the no-show fee applies at the next appointment.
