Woven Health Clinic in Farmers Branch is a nonprofit, low-cost clinic committed to making quality healthcare accessible. We offer affordable medical services, including primary care, chronic disease management, and preventive screenings, ensuring that cost is never a barrier to care. Patients can choose from our low-cost visit options or enroll in our Direct Primary Care program, which provides unlimited visits, same-day and next-day appointments, and transparent pricing.
Our comprehensive approach to healthcare goes beyond treating illness—we focus on overall wellness. Through programs like Mind Matters Wellness for mental health, our Heart Healthy initiative, and specialized women’s health services, we support patients in managing their health proactively. We also provide nutrition counseling with our registered dietitian, helping patients make informed choices to improve chronic conditions.
At Woven Health Clinic, we understand that healthcare is more than just medical visits. That’s why we address the broader factors affecting health, such as access to healthy food, transportation, and social support. Woven Health offers low-cost healthcare services for uninsured adults 18-64 years old and are low to middle income levels.

PLEASE NOTE – Due to the possible volume of requests, you may not receive a call back until 48-72 hours later.
We appreciate your patience as we work to assist all as quickly as possible.
How to schedule an appointment
To schedule an appointment, please call (972) 484-8444 during business hours. The Clinic sees patients by appointment only. For returning patients, some same-day appointments are available on a first-call, first-serve basis.
What to bring to your appointment
New patients, please bring the following to your appointment:
- All medications that you are currently taking
- The name, phone number and address of your preferred pharmacy
- The name and phone number of your previous physician
- Medical records from your previous physician — please have these faxed, emailed or mailed prior to your visit
- Medical records from any emergency room visits within the last 12 months
- A form of identification with your picture (driver’s license, passport, matriculate, student ID)
- Proof of income
- Completed new patient forms (click here to download in English)

Returning patients, please bring the following to your appointment:
- All medications that you are currently taking
- A form of identification with your picture (driver’s license, passport, or student ID)
- Medical records from any emergency room visits since your last appointment
Appointment costs and cancellation fees
First-time visit: $30.00
Regular appointment: $30.00
Well-women visit: $50.00-$65.00
Appointment no-show or cancelation: $25.00
Cancellation and reschedule policy
- We strive to be flexible and accessible for our patients, and recognize that sometimes patients will need to reschedule or cancel appointments.
- In order to best serve all our patients, scheduled appointments must be cancelled or rescheduled no less than 48 hours prior to the appointment during business hours.
- New patients who do not cancel or reschedule 48 hours prior to their appointment during business hours will be charged a $25.00 no-show fee.
- Returning patients who do not cancel or reschedule 48 hours during business hours prior to their appointment will be charged a $20.00 no-show fee at their next appointment.

All appointments must be canceled within 48 business hours prior to the date of the appointment. If the appointment is canceled with less than 48 business hours then the no-show fee applies at the next appointment.
