Woven Health Clinic Earns Three 2026 American Heart Association® Achievement Awards
The American Heart Association® has recognized Woven Health Clinic with three 2026 achievement awards for measurable results in blood pressure, diabetes, and cholesterol care.
These awards are earned, not nominated. They are based on adopting evidence-based care processes and on the health outcomes we actually deliver for the patients we serve across Farmers Branch, Carrollton, and the surrounding communities.
Target: BP™
Awarded for achieving a blood pressure control rate of 70% or greater among our adult patients, alongside evidence-based activity in the Measure Accurately and Act Rapidly pillars.
Target: Type 2 Diabetes℠
Awarded for strong HbA1c control among patients with diabetes, alongside 70% or greater adherence to a cardiovascular-focused clinical measure.
High blood pressure, diabetes, and high cholesterol are three of the most common and most manageable chronic conditions in our community — and three of the most likely to go untreated when care is hard to reach. Recognition like this reflects what our team sets out to do every day: make quality care accessible, and then make it work.
Awards presented by the American Heart Association® through the Target: BP™, Target: Type 2 Diabetes℠, and Check. Change. Control. Cholesterol™ programs.
