How to Schedule
To schedule an appointment, please call (469) 574-2022 during business hours. The Clinic sees patients by appointment only. For returning patients, some same-day appointments are available on a first-call, first-serve basis.
What to Bring to Your Appointment
New Patients
- All medications you are currently taking
- Name, phone number and address of your preferred pharmacy
- Name and phone number of your previous physician
- Medical records from your previous physician (faxed, emailed or mailed prior to visit)
- Medical records from any ER visits within the last 12 months
- Photo ID (driver's license, passport, matriculate, student ID)
- Proof of income
- Download new patient forms (English)
- Download new patient forms (Spanish)
Returning Patients
- All medications you are currently taking
- Photo ID (driver's license, passport, or student ID)
- Medical records from any ER visits since your last appointment
Appointment Costs
Cancellation & Reschedule Policy
- We strive to be flexible and accessible for our patients, and recognize that sometimes patients will need to reschedule or cancel appointments.
- Scheduled appointments must be cancelled or rescheduled no less than 48 hours prior to the appointment during business hours.
- New patients who do not cancel or reschedule 48 hours prior will be charged a $25.00 no-show fee.
- Returning patients who do not cancel or reschedule 48 hours prior will be charged a $20.00 no-show fee at their next appointment.
